Do you need help at home?
The Home and Community Care Program for Younger People is for people aged under 65 and Aboriginal or Torres Strait Islander people under 50 who need help with daily activities like:
Personal care & dressing
House cleaning
Nursing
Making meals or having meals delivered to you
Podiatry
Occupational Therapist
Dietician
Accessing the community and public transport
Respite for you, if you are someone’s carer
You may be eligible for these services if you:
Suffer from a chronic illness
Have short-term help needs
Are someone’s carer
A Living at Home Assessment takes place in your home when able to. It helps you look at ways to live in your home and still be active in your community. From the Assessment a Care Plan will be developed that meets your needs and goals.
You can refer yourself for an Assessment by calling the Pyrenees Shire Council on 1300 797 363 and ask for the Assessment Officer.
You can also talk to your doctor or any health service, friend or family member who can ring on your behalf.
